By signing below, the undersigned waives any and all rights to claims against the Boone County 4th of July Committee, its officials, sponsors, or volunteers, for any reason including but not limited to, loss of life, bodily injury, property damage, and/or loss that may occur during or as a result of participation in the Parade.

Acceptance of this application and participation in the Parade is subject to the discretion of the Boone County 4th of July Committee, who reserves the right to reject any application for any reason.
